What this scheme offers
Launched in 2019, MMJAY exists because Ayushman Bharat PM-JAY uses the 2011 SECC database to decide who's covered — and any household formed or genuinely impoverished after that census simply isn't on the list. Uttar Pradesh funds MMJAY itself to close that gap: it gives the identical ₹5 lakh per family per year cashless cover, at the same 1,500+ empanelled hospitals used for PM-JAY, to families the central list missed. A July 2021 government order specifically extended it to roughly 40.78 lakh Antyodaya Anna Yojana (AAY) ration-card families, and registered construction workers under the BOCW board are separately eligible too. In practice, PM-JAY and MMJAY together are marketed in UP as one combined 'Ayushman UP' — most residents never need to know which of the two is actually paying their hospital bill.
Who is eligible
- Resident of Uttar Pradesh with a ration card, not covered under the central PM-JAY (SECC 2011) list.
- Antyodaya Anna Yojana (AAY) ration cardholders are specifically covered (since July 2021).
- Registered Building & Other Construction Workers (BOCW) are separately eligible.
- No cap on family size or age; pre-existing conditions are covered from day one.
Documents required
- Ration card
- Aadhaar card
- Proof of AAY status or BOCW registration, where applicable
How to apply
- Check your eligibility at ayushmanup.in or the beneficiary lookup on the Ayushman Bharat app using your ration card or Aadhaar.
- If not already listed, visit your nearest Jan Seva Kendra (CSC), government hospital, or ask an Arogya Mitra (hospital help-desk staff) to verify your status.
- Once confirmed eligible, your Ayushman/Golden Card is issued — this is what you present at any empanelled hospital.
- At the hospital, present your card; treatment covered under the scheme is cashless — you shouldn't be asked to pay upfront for a covered procedure.
- Card issues or approval delays can now be resolved at the district level through your local Chief Medical Officer (CMO) office.
